We are so thrilled you’d like to join us in loving on our caregivers at the Fun & Feisty Caregiver Summit. This is a preliminary application to see if you would be a good fit for our Summit. Please fill out the form so we can get you get to know you better. Once you hit submit, we will review your application and let you know our decision. Thank you!
Special needs parents, elder care, spousal care, or other. Preference will be given to those with more lived-experience in the caregiving community. Please provide as many details as possible.
Thank you for participating in our Fun & Feisty Caregivers. Please complete this form to allow for your promotional materials and advertisement to be created. Participation with Fun & Feist Caregivers may include an electronic recording of your advertisement, herein referred to as the “Advertiser" Advertisement with Katy Kaumeyer & Michelle Slaney-Trovato, herein referred to as "Host". Advertiser understands that for educational, promotional, and commercial applications, Host will make Advertiser’s advertisement available at no charge to event registrants and attendees. 1. Advertiser hereby grants to Host the right to use his or her name, his or her company name, voice, appearance, likeness, image, words, participation, and title or professional designation, in whole or part, in these recordings and/or compilations and in any correspondence, publicity, advertising, promotional, or other derivative materials based on them. Host has no obligation to make use of the rights granted herein. 2. Advertiser hereby grants Host the unconditional and irrevocable right to reproduce the Advertisement in any format, and to distribute, sell, prepare derivative works, advertise, and publicize the Advertisement without compensation to Advertiser. However, Advertiser shall also retain the right to use his or her own Advertisement for any purpose. However, Advertiser shall also retain the right to use his or her own Advertisement for any purpose. 3. Advertiser, upon written request to Host, shall be provided with a copy of his or her Advertisement in electronic format. 4. Advertiser agrees to promote to the relevant segments of their database and all available social media outlets during the stated promotion period. Promotional period will be one year from date accepted by Host. Advertiser also agrees to make at least 1 (one) promotional post and email during the active dates.
